There is a permanent 16 hour per week position based in Porirua and there is further opportunity to work on-call across our three community bases and IPU to provide leave cover and visit support for Social Workers who work in the Wellington, Porirua and Paraparaumu communities.
In this role, you will be part of a highly committed team with responsibility for providing specialist palliative care to patients and their families and whānau. The role includes providing specialist palliative care social work to patients and their families as well as education and bereavement services.
Applicants need to be registered, have a social work qualification, and have a sound theoretical knowledge and appreciation of the grief process and the special nature of working with people with a life threatening illness.
An understanding of and commitment to the multidisciplinary team approach to patient care is essential and experience working in the community would be an advantage.
