Mary Potter Hospice is seeking to appoint an experienced Social Worker to join our mulitidisplinary team supporting Aged Residential Care (ARC) providers by providing clinical social work support and leadership to ensure co-ordinated and integrated care that enhances seamless patient care for those patients with complex palliative needs. The Social Worker will strengthen the interface between generalist and specialist palliative care services by providing specialist advice and co-ordination and promoting a cohesive and consistent quality of palliative care within the ARC Settings.
Applicants need to be registered, have a social work qualification, and have a sound theoretical knowledge and appreciation of the grief process and the special nature of working with people with a life threatening illness.
An understanding of and commitment to the multidisciplinary team approach to patient care is essential and experience working in the community would be an advantage.
